Mission: To provide primary health care, health education,
and screening to the un-and underinsured, medically vulnerable
residents of Grand Rapids’ Northeast Side and the surrounding
communities.
Our goal is to provide our patients with the health care
and information they need to make decisions about their health
and assist them in accessing the resources and services needed
to achieve and maintain optimal health.
Established 1996 as a unique partnership between Saint Mary’s,
Saint Alphonsus Parish, and the Creston/Belknap Neighborhood
Associations, Catherine’s is a community based health
center dedicated to wellness and preventative care for the
area’s working poor. We have a large elderly patient
population for whom we are their only source of medical and
social services care. Catherine’s is strategically placed
in a federally designated Health Professional Shortage Area.
We’ve grown from a preventive health care clinic with
approximately 1,000 patients in 1998 to a full service medical
center providing services to over 5,700 persons in 2001
Staff includes one half-time physician, one full-time R.N.
and neighborhood health coordinator, one half-time medical
assistant, volunteer physicians, nurse practitioner’s,
receptionist, and office personnel.
Special recognition
- 2001 recipient of the prestigious Douglas A. Mack Award
for Community Collaboration from the Kent County Health
Department
- 2002 recipient of Trinity Health’s award for demonstrating
exceptional health care programming, fostering community
collaboration, and enhancing the ministry of Trinity Health.
