Family Practice
Our Physician and Physician Assistants have been trained
to consider the whole person, their family, and the community
in which they live, not just an illness or body system. Our
support staff of nurses, radiology and laboratory technicians,
billing and collection specialists, and scheduling coordinators
are committed to making your visit to our office a satisfactory
experience for you. All of our practitioners and most of our
support staff are fluent in both Spanish and English. We have
access to interpreter services for other languages as needed.
As part of the Saint Marys Mercy Medical Center Community
Health Care System, the providers at Sparta Health Center
have access to a wide array of specialists and services.
When our office is not open, the telephone will be answered
by our answering service. If you have a life-threatening emergency,
CALL 911. For less serious concerns, you may call Advantage
Health After-Hours Center when they are open, or go directly
to the Emergency Department at Saint Marys. We prefer
that you use these facilities when we are not open so that
your personal provider can coordinate your follow-up care.

When you need to make an appointment
Please call our office to make an appointment. Our scheduling
staff will ask you some questions about the problem you are
having in order to allot an appropriate amount of time for
your visit. Each member of our staff is aware of the importance
of your time and of our providers goal to have adequate
time to take care of each patient. Confidentiality of patient
information is a prime concern in our office, and your privacy
will be protected at all times.
When you need services outside of our office
Our referral coordinator will help you and your primary care
provider find an appropriate source for the services you may
need. If you are a member of a managed care plan, the coordinator
will be able to make a referral within the approved panel
of providers or help you obtain permission to go outside that
plans network.
When you need a hospital
Because we are part of Advantage Health and Saint Marys
Mercy Medical Center, when you need to be admitted to a hospital
you will be sent to Saint Marys Hospital in Grand Rapids.
Sparta Health Center patients are admitted to Saint Mary's Health Care unless there is a compelling medical
reason to go elsewhere. Dr. Schneider delivers babies, and
attends all of our patients who are admitted to Saint Mary's Health Care. Follow up care and coordination is provided
by Dr. Schneider and the support staff at Sparta Health Center.
When you need a prescription filled
First, read the label on your medication. If there are additional
refills indicated on the label, go directly to the pharmacist
who originally filled the prescription. If not, call our office
and leave a message on the prescription refill message machine.
If your provider authorizes a refill, it will be called to
your pharmacy by the end of the next business day. If there
are questions regarding your refill, we will call you.
Paying for your services
We participate with many health plans such as Care Choices, Priority
Health, Blue Care Network, and PPOM, as well as Medicare, Medicaid,
Blue Cross Blue Shield, and other commercial insurance carriers. There
is a sliding payment scale for patients who have no insurance, or
whose insurance is inadequate. Members of the billing office are available
to explain the eligibility criteria. |
