Medical Records

The Medical Records/Transcription Department processes, stores and retrieves all inpatient records, as well as records for Emergency Department patients, and most outpatients, excluding Clinic, Outpatient Lab, Outpatient Rehab and Dialysis.

Services

  • Maintain medical records for ongoing patient care
  • Forward records to physicians' offices, insurance companies and hospitals
  • Respond to subpoenas for medical records
  • Review records for completeness, make incomplete records available to physicians for completion
  • Transcribe dictated reports (H&P, Operative, Discharge Summary, Emergency Department, Consults, PMU)
  • Code medical records for billing/clinical database
  • Provide abstract medical records for clinical database
